Scholarship Program
Green Hill Therapy is pleased to invite you to apply for financial assistance through the Green Hill Therapy Scholarship Program.
The overriding purpose of Green Hill is to ensure that children throughout our community, particularly special needs children, receive the support and proven intervention they need to reach their full potential, regardless of their ability to pay for those services.
The goal of the Green Hill Scholarship Subsidy Program is to provide temporary financial assistance in times of hardship to enable children to receive the benefits of the various therapies we offer.
The Scholarship Program is a direct result of the generosity of individuals, corporations and foundations who have supported Green Hill.
Listed below are the items we will need from you to complete your application package:
1. The Green Hill Therapy Financial Need Form.
2. Three years of your most recent tax returns.
3. An itemized listing of the annual medical expenses directly related to the applicant. This number must not include any other members of the family.
4. Proof of insurance or proof of denial.
5. The dollar amount of any SSI benefits the applicant receives.
